I hope this message finds you well. I am reaching out to formally request an evaluation to determine whether [my child / I] qualify for educational accommodations through a 504 Plan or Individualized Education Program (IEP).


[My child has / I have] been experiencing challenges in the classroom that are impacting [his/her/their/my] ability to fully access the curriculum. Based on these ongoing difficulties, I would like to explore the possibility of implementing accommodations and/or support services to better meet [his/her/their/my] learning needs.
